Sheffield - MH Hospital Discharge - £30/hour

There is an opportunity for a temp to join our mental health hospital discharge team in Sheffield City Council, pay @£30 per hour, start date asap for 3 months minimum.

Based at Michael Carlisle Centre and across SHSC Trust work sites with 1-2 days a week from home if wanted.

The candidate will need to be an experienced social worker with a mental health background.

Core tasks and duties as follows:

Undertaking social care assessments and delivering timely care packages for hospital inpatients prior to discharge
Some mental capacity assessments, BI decisions and safeguarding as required
Attending 117 meetings, ward rounds, professional meetings
Facilitating learning opportunities for ASYEs through shadowing
Participation in duty rotaSuccessful candidate will be welcomed and integrated into the team and offered regular supervision and support from a manager.
