The service you will work in….

This service is a joint venture with community NHS team, supporting people with dementia who are living in their own homes. We aim to provide person-centered care planning and support to people with dementia. The service operates across the entire borough of Hounslow and provides a seamless service across all elements.

We provide practical, person-centered support that promotes wellbeing, independence, dignity and respect. Working with people to identify their strengths, personal goals and aspirations and develop holistic, outcomes-focused plans to support people as they move towards those goals, and prevent people with dementia being admitted to hospital.

We empower people to speak up for themselves, understand their rights and have greater control over their lives and provide carers with the opportunity to take time out from their caring responsibilities to look after their own wellbeing. We will also work alongside befrienders, volunteers, and peer supporters to deliver activities that promote independence and reduce reliance on services. 

A day in this role looks like….

As a Dementia Coordinator at Hestia, you will play a crucial role in our Mental Health and Complex Needs Service. Your contributions will directly influence the lives of our service users, guiding them towards a path of empowerment and independence.

A day will never look the same and some of the things you can expect to do/be responsible for are:

Supporting people with dementia and their carers in their homes.

Social work student supervision- training will be given.

Develop high quality co-produced care plans with the person with dementia.

Provide holistic integrated support work to people with dementia.

Undertake assessments of risk with due regard to safeguarding.

To ensure that the communication needs of service users are met.

To have a good knowledge of entitlements and eligibility for those living with dementia and their carers, to services such as benefits and respite.

To identify the needs of carers and signpost effectively.

Effective working as part of a multi-disciplinary team to meet the needs of clients and reduce hospital admissions for those living with dementia. 

Continuously update knowledge on dementia to maintain status as Subject Matter Expert.

We are looking for candidates who bring a combination of passion, expertise, and dedication to the table. To excel in this role, you should possess:

Good knowledge of how dementia affects people and their carers.

At least 2 years of experience of working with service users in a social care or health care sector (preferably older people).

In depth understanding of support available to older adults with illnesses such as dementia, such as benefits and social support to maintain independence.

The ability to work in a way that promotes and maintains dignity.

Experience of providing services to people with dementia in either a health or social care environment.

Ability to co-produce care plans with the person with dementia, their families and immediate carers.

Excellent communication skills.

Knowledge and understanding of safeguarding issues and ability to address them appropriately.
